Proxy Access Request

Thank you for your interest in MyMarsden, an easy-to-use internet tool that provides quick and secure online access to health information. If you have received a proxy invitation to MyMarsden and do not yet have a MyMarsden account, please complete and submit the following form for approval. Once your request has been approved, we will email your activation code within 7 business days. We will contact you if we have any questions regarding your information.

Please note that access to a child’s record through MyMarsden can only be granted to a Parent or Legal Guardian after discussion with the child's care team.
